Fix for Kentucky E-Claim and Century Fields

Corrected Y2K compliance for state of KY. All date fields where indicating the century except for date of birth. This only affects state of Kentucky.
Solution: install this patch and send the batch again.
NO need to refile.

Physicians A/R Report Selection Fixed

Corrected problem with Physicians AR Receivables.
The report would not find any records if a
restriction for payer code was specified.
